Florida High School Athletic Association Sports Physical Forms

If your child is participating in school athletics, the Florida High School Athletic Association requires the following forms.

Please Print and Complete Before Your Appointment
Bring these completed forms to your child’s sports physical:

• • Florida High School Pre-participation Physical Form:  EL1_ECG_Screening
  • Florida High School Pre-participation Physical Form:  EL1 – 2S_Medical eligibility supplemental form
  • Florida High School Pre-participation Physical Form:  EL2_Participation Physical Evaluation

>>  Having these forms filled out in advance helps us complete your child’s physical efficiently and avoid delays.

Do Not Bring to the Appointment

• • EL3 Consent and Release from Liability Certificate

>> This form is required by the school, but it does not need to be brought to the medical visit.
